Assessments help establish the right level of care for older adults who are struggling to independently manage activities of daily living (ADLs) through an evaluation of functional abilities, cognitive health, personal care needs, and overall health. Assessments can serve multiple purposes such as determining eligibility for long-term care insurance (LTCi) claims or forming the foundation of a personalized plan of care tailored to your needs.

We conduct our assessments through our Clinical Field Team Partners, a nationwide network of clinicians that have conducted over one million assessments both in-person and telephonically.
