How It Works

How do I get my Care Plan?

Schedule a one-time care evaluation at your convenience

We've made it easy for you to let us know what days and times work for you and your family or friends. The evaluation is virtual, so you can have your supporters attend if you wish.

Meet with a licensed nurse for your evaluation

During your one-time virtual visit, your nurse will ask questions to evaluate physical and cognitive abilities. The nurse will also want to know about your goals, activities, and preferences.

Receive your personalized Care Plan

We'll email you your Care Plan, outlining the care, equipment, and resource options that may be most helpful to you so you can safely live the life you want.
