"The cornerstone of our service is an individualized care plan drafted by one of our nurses during a home visit. We absolutely believe that care should be person-centered and completely tailored to the client's individual needs. We update our care plans based on changes in condition reported to us by our care professionals or observed by our nurses during their regular quality assurance home visits."
